GUSTO TECHNOLOGY

1. PURPOSE AND SCOPE

Pursuant to Article 11 of the Law No. 6698 on the Protection of Personal Data ("KVKK"), data subjects have the right to apply to the data controller regarding their personal data. This Application Form has been prepared in accordance with the Communiqué on the Procedures and Principles of Application to the Data Controller (“Communiqué”) to enable you, as a data subject, to apply to Gusto Teknoloji Haberleşme Telekomünikasyon Sanayi ve Ticaret Limited Şirketi ("Gusto Technology") and exercise your rights regarding your personal data.

2. DATA SUBJECT INFORMATION

In order to process your application and communicate the result to you, please complete the information below accurately and completely.

The information you provide here will only be used for the purpose of evaluating and finalizing your application and will be retained for a reasonable period in case of any complaint or dispute.

4. EXPLANATION REGARDING YOUR REQUEST

To help us process your request more effectively, please explain in detail the requests you have marked above. Gusto Technology shall not be held liable for any errors or faults arising from missing explanations.

5. METHODS OF APPLICATION

To submit your application, you must first complete this Form. After signing the Form, you may submit a signed copy by:

• Delivering it by hand by presenting your ID to Atıf Bey Mah. 10 Sk. No:2/1A Gaziemir İZMİR/TURKEY

• Sending it via registered mail to the same address, or

• Sending it to [email protected] using your secure electronic signature, mobile signature, or your email address previously registered in our system.

These application methods are established to verify that the applicant is the rightful data subject. The Personal Data Protection Board (“Board”) may introduce other application methods in the future. In cases where a third party applies on behalf of the data subject, a notarized power of attorney or documentation proving guardianship must be included with the application.

6. FINALIZATION OF THE REQUEST

We will respond to your application as soon as possible and no later than thirty (30) days after receipt. Gusto Technology may contact you via your registered email or phone number to verify your identity.

If your application contains incomplete or inaccurate information or documents, we may request that these be completed or corrected. The 30-day response period will be paused until the required information or documents are provided.

7. SIGNATURE OF THE DATA SUBJECT

As the data subject / authorized person, I am submitting this application regarding personal data processing activities under the KVKK. I declare that all the information and documents provided, including this Application Form, are accurate, complete, up-to-date, and belong to the data subject; and I accept that Gusto Technology shall not be held responsible for any delays or errors resulting from incorrect or outdated information.
